1. EXECUTIVE SUMMARY

1.1 Title Page


[The Executive Summary will have a title page containing the following information:

[The Technical Proposal will contain a table of contents on a separate page after the title page.]

2.3 Technical Requirements.

[Discuss the ability of the Respondent to meet the requirements specified in the Statement of Work. Such discussion will include, at a minimum, the methodology, equipment, personnel, Subcontractors (if any) and accuracy involved in such testing, measurement or verification.]

[ Alternate Proposals only: provide a short description of how the work proposed is different from what is contained in the Statement of Work.]

2.4 Facilities

2.4.1. Plant.

[For each plant that will be involved in the Services, provide the following information in the following format.]

General Plant Information: [Generally describe the plant, including its location, its size (e.g. area of manufacturing, office and laboratory space) and other pertinent characteristics.]

Plant Functions for LBT: [Describe the functions involved in the Services that will be conducted at the plant.]

Current Plant Usage: [Describe the current use of the plant. Include the type of products currently produced and processes utilized in their production.]

2.4.2. Equipment.

General Equipment Information: [Generally describe the equipment, including its proposed use in performing the Services, its operating and performance characteristics (e.g., capacity, accuracy, etc.), its location (during its involvement in the Services), and other pertinent characteristics.]

Current use of the Equipment: [Describe the current use of the equipment.]

Contingency Plans:

[Briefly describe what actions will be taken to replace, or substitute for, this equipment if it becomes unavailable. Include a summary (providing the same information specified for the original equipment) of possible alternate equipment, if any.]

2.5 Subcontractors.

[Provide the name and address, including the name and telephone number of the principle contact, of all Subcontractors that will or may be used by the Respondent in any aspect of the Services. Subcontractors include: (a) consultants and advisors that are not directly employed by the Respondent or its parent organization; and (b) persons or organizations directly involved in performing the Services, that are not directly employed by the Respondent or its parent organization. Also discuss the functions and capacity in which each of the Subcontractors will serve, as well as the actions that will be taken to insure that such functions and capacities will be performed if the Subcontractor becomes unavailable.

Finally, specify the primary suppliers to be used in performing the Services, including a description of the products that each will provide. Also discuss the actions that will be taken to insure that such products will be obtained if the supplier becomes unavailable.]

2.6 Production Experience.

3. MANAGEMENT PROPOSAL

3.1. Title Page.

3.3.3. Names of Contacts.

3.3.3.1. Technical Matters. [The name, address and telephone number of the person designated by the Respondent to answer questions regarding technical issues.]
3.3.3.2. Management Matters. [The name, address and telephone number of the person designated by the Respondent to answer questions regarding management issues.]
3.3.3.3. Financial Matters. [The name, address and telephone number of the person designated by the Respondent to answer questions regarding financial issues.]

3.3.4. Entity Structure.
3.3.4.1. Type of Entity. [Private, partnership, corporation, quasi-governmental, etc., as appropriate.]
3.3.4.2. Nation of Organization. [The nation under which laws the entity is formed.]
3.3.4.3. State of Organization. [If formed under U.S. law, the appropriate state.]
3.3.4.4. Date of Entity Formation. [Date of legal formation of entity.]

3.3.5. Name of Parent. [Formal name.]
3.3.6. Address of Parent. [Primary office address for correspondence.]
3.3.7. Parent Entity Structure.

3.3.7.1. Type of Entity. [Proprietorship, partnership, corporation, etc., as appropriate.]
3.3.7.2. Nation of Organization. [The nation under which laws the entity is formed.]
3.3.7.3. State of Organization. [If formed under U.S. law, the appropriate state.]
3.3.7.4. Date of Entity Formation. [Date of legal formation of entity.]

3.4. History of Respondent.

[Briefly discuss the history of the Respondent and its parent organization, if appropriate.]

3.5. Project Management.

3.5.1. Organization Structure.

[Include organizational charts for the Respondent, as a whole, and for management of the Services specifically. Illustrate how the organizational structure for the Respondent will interact with the organizational structure for the Services, for purposes of involving Respondent management personnel in management of the Services. Each area of responsibility should be set out separately, and should include the title and name of the responsible person(s).]

3.5.2. Work Breakdown Structure.

[Present a work breakdown structure for providing the Services. The work breakdown structure should be presented with sufficient detail to determine the reliability of schedule and costs presented by Respondent elsewhere in the Proposal Documents.]

3.5.3 Assurance Programs

[Briefly describe any of the Programs that might be in place at your facility and how they would be applied to the services to be provided:

3.6. Project Schedule.

3.6.1. Proposed Schedule.

[Provide the following information regarding proposed scheduling of the Services. The milestones specified in the table should correspond to the work breakdown structure contained in Section 3.5.2, above.

The schedule shall list all actions which are planned for the manufacture, inspection, testing and transport of the units. Constraints on procurement or deliveries shall also be identified.]

3.6.2. Attachment Contingencies.

[Describe the contingency plans, if any, that the Respondent proposes to implement in the event that any aspect of the schedule presented in Section 3.6.1, above, begins to slip.]

3.7. Personnel.

3.7.1. Personnel by Discipline.

[List the number of persons currently employed by the Respondent (or applicable division of the Respondent that will be involved in the Services). List each person only once, by primary discipline.]

3.7.2. Proposed Personnel for Services. [List the number of persons that will eventually be directly involved in the Services for each discipline, whether or not such persons are currently employed by the Respondent. List each person only once, by primary discipline.]

4. COST PROPOSAL

4.1. Title Page.

[The Cost Proposal will have a title page containing at least the following information:

LBTC preference is for a list of milestones and progress payments, but other payment terms will be considered.]

4.2 Table of Contents.

4.3. Payment Terms and Conditions

5. PROPOSAL SIGNATURE
